(Note: If you request it upon submission, you can change it to the new-style kanji.)
Common mistake points in marriage registration form 2: Either Japanese era or Western era is fine
For the date of birth under the name, it is acceptable to use either the Japanese era format "Showa ○○ year" or "Heisei ○○ year" or the Western format "19○○ year"*
Depending on the marriage registration form, it may already indicate Heisei or Showa.

You should write the same address as the one on your resident certificate at the time you submit the marriage registration*

2. Have witnesses sign on both the main marriage registration form and a duplicate marriage registration form.
3. While looking at the draft marriage registration form, fill out the main one. (Do not write on the duplicate.)
4. When submitting, bring both the main marriage registration form and the duplicate where the witnesses have signed. If there are deficiencies in the main marriage registration form, use the duplicate as the main one, rewrite it, and submit it again.
